“Will there be forms to fill out when I arrive?”
Yes, there will. The good news is that you do not have to fill them out in the waiting room. Here is how to make your first visit go smoothly, from the phone call to the front desk.
When you call: have your insurance information ready
To make an appointment, we need your insurance information at the time of your call. Please have your insurance card in front of you when you phone (713) 722-0111. Without it, we are not able to book the appointment, and we would rather not have you call twice.
Fill out your forms at home
You can download our forms from our new patient forms page, fill them out at home and bring them with you. It saves time on the day of your visit.
What to bring
- Your completed new patient forms
- Your insurance card
- Your driver’s license
We will make a copy of your insurance card and driver’s license when you arrive.
Where to go
Dr. Berzin sees patients at our Medical Center office in Scurlock Tower, 6560 Fannin Street, Suite 1804, Houston, TX 77030, on Wednesdays. He also sees patients in Kingwood every other Thursday, 3:00 to 5:00, by appointment only.

Cuando llame: tenga lista la información de su seguro
Para hacer una cita, necesitamos la información de su seguro en el momento de la llamada. Por favor tenga su tarjeta del seguro a la mano cuando llame al (713) 722-0111. Sin ella no podemos programar la cita, y preferimos que no tenga que llamar dos veces.

Puede descargar nuestros formularios de nuestra página de formularios para pacientes nuevos, llenarlos en casa y traerlos con usted. Así ahorra tiempo el día de su cita.

El Dr. Berzin atiende pacientes en nuestra oficina del Centro Médico, en Scurlock Tower, 6560 Fannin Street, Suite 1804, Houston, TX 77030, los miércoles. También atiende en Kingwood cada dos jueves, de 3:00 a 5:00, solo con cita.
